USCIS Immigration Physicals
A Civil Surgeon designated by the U.S. Department of Homeland Security will conduct your immigration medical examination and complete Form I-693
What to Bring
- Photo ID (U.S. Driver’s License, State ID, Passport, Military ID OR Green Card)
- Medical & vaccination records, if available, including:
> Influenza (Flu)
> MMR
> Varicella (Chickenpox)
> Hepatitis B
> Tdap
> Polio
What To Expect
- Initial Visit: Medical exam, lab draws, urine test, blood test for TB, immunity titers (as needed for MMR, varicella and Hepatitis B) and vaccination review.
- Follow-Up Visit (2 weeks later): Civil Surgeon completes Form I-693, places it in a sealed envelope for USCIS submission. Do not open the sealed envelope – USCIS will reject it if opened.
- $500 (Includes medical exam, blood test, urine test, and vaccination record review. Cash and Card Accepted
